This module is part of a series value-based payments for behavioral health providers. Population health management is a critical component of value-based payment arrangements, as it requires providers to focus on improving and maintaining the health their entire patient population across the full continuum of care. This module introduces the core concepts of population health management. After taking this course, participants should be able to: identify the foundational elements of population health management and identify the key components to adopting and implementing population health management within their organizations.
